Providing your bank account number for the 'health costs assistance payment'
If you are receiving an occupational disability benefit (‘arbeidsongeschiktheidsuitkering’), you will most likely face higher health costs. We will help you with these expenses by transferring a 'health costs assistance payment’ (‘Tegemoetkoming arbeidsongeschikten’) to your bank account each year.
Which criteria do you need to meet to qualify for the payment?
- You are receiving a WIA, WAO or WAZ benefit on 1 July of this year. Additionally, you are unable to perform more than 35% of your work duties due to your occupational disability.
- You are receiving a Wajong benefit on 1 July of this year.
Please note: You will not be entitled to the payment once you reach the state pension age (‘AOW-leeftijd’) as your WIA, WAO, WAZ or Wajong benefit will automatically end then.
What you need to do
We need to know which bank account number the payment should be transferred to. This is why we sent you a letter requesting this information. Please provide your bank account number by completing and returning the form that was enclosed with the letter.
Should you prefer to use the English version of this form, you can do so by completing the form online. You will be able to access this form by logging in with your DigiD account or a valid European electronic ID (‘Europees erkend inlogmiddel’).
Please only fill out the form if you have received a letter from us specifically asking you to do this.
When you will receive this payment
You will receive this payment each year in September.
