Having a medical examination in your country of residence or in the Netherlands
We need to assess your health to determine whether you will qualify for a sickness (‘Ziektewet-uitkering’) or WIA benefit. Or, if you are already receiving one of these benefits, we need to determine whether you are still entitled to it. This is why you have received a letter inviting you for a medical assessment. You can choose to have this examination in your country of residence or in the Netherlands.
Who should fill in the form?
This form is intended for people living abroad who might qualify for a sickness or WIA benefit. You will only need to fill in this form if you have received a letter from us asking you to do so. Additionally, 1 of the following situations must apply to you:
- You have notified us of your illness and have requested a sickness benefit.
- You have asked us to reassess your WIA-benefit (‘herbeoordeling WIA-uitkering’).
How to inform us of your preference
Let us know where you would like to have the medical examination by completing the form Preference for medical examination in your country of residence or in the Netherlands. You will need either a DigiD account or a valid European electronic ID (‘Europees erkend inlogmiddel’) to access this form.
What happens next?
- If you would prefer to have a medical examination in your country of residence, the benefits organisation in that country will contact you directly.
- If you would prefer to have a medical examination in the Netherlands, we will send you an invitation for an appointment as well as information about how to claim travel and accommodation costs.
