Completing the life certificate form
If you are receiving an occupational disability benefit while living abroad, you will need to complete the ‘life certificate form’. You must do this each year in order to keep receiving your benefit.
How to complete the form
We will send you the life certificate form once a year. Make sure you complete this form within 6 weeks of receiving it.
Proof of your identity
You must submit a copy of a current proof of identity along with your form. Make sure your citizen service number is included on this document and that your photo and personal information are clearly visible.
