Requesting return-to-work support
As an employer, we expect you to do whatever you can to support your employee’s return-to-work process. If your employee’s contract is expected to end within the first 2 years of illness, UWV can offer financial support.
In what types of situations can you request return-to-work support?
- Your employee is still too ill to work when their employment contract ends.
- You will have to pay your return-to-work service provider (‘re-integratiebedrijf’) more than 70% of what your employee will be earning up to the end of their contract.

Please note: If you are self-insured, you are personally responsible for your employee’s return-to-work process. This means you will not be able to request return-to-work support.
How to request return-to-work support
To request return-to-work support, complete the ‘Request return-to-work support form’ (‘Verzoek participatie in re-integratie’).
You will need to submit the following documents along with the form:
- a copy of the Evaluation (‘Probleemanalyse’), the Action plan (‘Plan van aanpak’) and, if applicable, any changes to the Action plan or Evaluation (‘Bijstellingen’)
- a proposal for an initial return-to-work plan along with an estimate of the costs
- a recent payslip
If you do not send us these documents, we will not be able to process your request. You can send the form and the documents to the address specified on the form.
What happens next?
We will send you a confirmation letter within 2 weeks of receiving your form. If you want us to be involved in your employee’s return-to-work process, we will first need to check your return-to-work plan.
If you plan to hire a return-to-work service provider, we will need to check whether this provider meets our quality criteria.
