Please note: You cannot impose a penalty without a decision from us. You can request this decision when requesting an evaluation, or at a later date.

In the following situations, you may impose a penalty on your employee or former employee:

  • failing to provide a valid ID
  • obstructing inspections or make them impossible
  • failing to report sick on time, missing appointments without a valid reason or impeding medical examinations
  • not seeking medical assistance on time or ignoring their physician’s advice
  • providing false, late or incomplete information about work, income, change of address if moving abroad or any changes that may affect their sickness benefit
  • not putting enough effort into finding a suitable job
  • intentionally causing or prolonging their incapacity for work and hindering their recovery

To find out if you can impose a penalty on your employee or former employee, submit a request via Digipoort. If you prefer, you can complete the evaluation form (‘Beoordeling maatregel Ziektewet-uitkering’) and email it to info.erzw@uwv.nl.

You will need to provide the date on which your employee or former employee first became ill. Please also have the following details close at hand:

  • your payroll tax number (‘loonheffingennummer’)
  • your self-insured contributions code (‘sector en risicopremiegroep’)
  • your employee or former employee’s name, citizen service number (‘BSN’) and date of birth
  • your employee or former employee’s average daily income (‘dagloon’)
  • any relevant information and documents we might need to evaluate your request

The current cost of this service is 39 euros.

You can expect our evaluation within 4 weeks of receiving your request. You will also receive a letter from us with information on how to pay for it.
