Reporting a change in your health

To inform us of a change in your health, use the ‘Report a change in your health’ form that is intended for the specific benefit you are receiving:

for a WIA benefit, use: Report a change in your health (WIA)
for a WAO benefit, use: Report changes in your health (WAO)
for a WAZ benefit, use: Report changes in your health (WAZ)
for a Wajong benefit, use: Report changes in your health (Wajong)

The types of situations in which you should inform us of a change in your health include:

if your health has improved or worsened
if you have been admitted to a hospital, nursing home or other healthcare facility

Please note: If your health improves or worsens, we will need to re-assess your situation to see whether any changes need to be made to the benefit you are receiving.

If you are living abroad and need to report a change in your health, use the same benefit-specific forms that you would in the Netherlands (see the section above ‘How to inform us of changes in your health’).
