To re-apply, complete the Change of circumstances form for those receiving an occupational disability benefit (‘Wijzigingen doorgeven WIA,WAO, WAZ, Wajong’). You will need a DigiD account to do this.

You must meet 1 of the following criteria if you wish to re-apply for a Wajong benefit:

• Your Wajong benefit ended on or after 2 January 2016 and your health has worsened since then. Plus you have not yet reached retirement age.
• You cancelled your Wajong benefit more than a year ago.
• You have experienced changes in your personal situation, work or income.

You will be able to re-apply for a Wajong benefit if you are moving back to the Netherlands and your previous Wajong benefit ended when you moved abroad. To re-apply, please call UWV Telefoon Werknemers (the UWV employee helpline).

Your previous Wajong benefit might have ended because you were imprisoned for more than 1 month. If you are being released, you will be able to re-apply for a Wajong benefit. To do this, send a letter and your ‘Discharge from prison certificate’ to the following address:
