The ‘PD U1 Form’ lists the periods in which you worked as an insured employee in the EU/EEA or Switzerland. This is important as your employment history in these countries counts towards your general employment history and will affect your chances of receiving an unemployment benefit.
If you are applying for an unemployment benefit in the Netherlands and need a ‘PD U1 Form’, contact the benefits organisation in the country you worked in before coming to the Netherlands.
If you are applying for an unemployment benefit in the EU/EEA or Switzerland and worked in the Netherlands in the past, UWV will provide you with a ‘PD U1 Form’. To get this ‘PD U1 Form’, complete the online PD U1 application form.
You will need the following when completing the form:
- a DigiD account or an electronic ID that is valid in the EU
- a citizen service number
- your last employer’s address
Once you have done this, you should receive the ‘PD U1 Form’ 6 – 8 weeks later. Do not apply for the form any earlier than 4 weeks before your employment ends.
