If the request is for you:

  • your citizen service number ('BSN')
  • your medical report ('medisch rapport')
If the request is for a child:
  • the authorisation to act on the child’s behalf ('machtiging')
  • the citizen service number of the child
  • the medical report of the child

Please complete the request form ('Aanvragen voorziening'). You will need a DigiD account to do this. Make sure to tell us what you need a communication support worker for. You will need to make a new request for every specific situation.

Fill in the form online or, if you prefer, contact our helpline ('UWV Telefoon Werknemers') and ask for a hard copy. After completing the hard copy, send it to us at the following address:

Upon receipt of your request, we will send you a confirmation.

You will receive our decision within 8 weeks of us having received your form. However, we aim to let you know our decision within 10 working days. If we need more information from you, we will let you know.
