    Applying for a work capacity evaluation

    If you have an occupational disability, you might be entitled to extra support. The type of support you will be entitled to depends on your situation. If you are interested in receiving extra support, you will need to apply for a work capacity evaluation (‘beoordeling arbeidsvermogen’) first.

    How to apply for a work capacity evaluation

    To apply for a work capacity evaluation, complete the online Work capacity evaluation form. You will need either a DigiD account or a valid European electronic ID (‘Europees erkend inlogmiddel’) to do this.

    Documents that you will need to send along with your application form

    • reports about your return-to-work process 
    • reports from your job coach
    • recommendations that you receive independent living care (‘Wmo-indicatie’)
    • recommendations that you receive long-term care (‘Wlz-indicatie’)
    • reports on how you are doing at school or your internship
    • documents proving you attended or are attending a special educational needs school (if applicable). An example of such a document would be a ‘statement of special educational needs’, known as a ‘toelaatbaarheidsverklaring’ in Dutch.

    If you have not attended a special educational needs school and are applying for a Wajong benefit or a recommendation for access to work, please submit the following documents:

    • any medical records from your GP and/or specialist
    • any medical recommendations
    • any reports by a developmental-behavioural paediatrician if you saw one
    • any reports by a psychologist if you saw one

    Please note: If you upload these documents, we will be able to process your application more quickly.

    If you do not want to apply online

    If you do not want to apply for a work capacity evaluation online, call UWV Telefoon Werknemers (the UWV employee helpline). Ask them to send you a hard copy of the form. Please keep your citizen service number close to hand so we can help you quickly and efficiently.

    What happens next?

    We will confirm that we have received your application within 5 days of you sending it. You might also be invited to an appointment with an UWV physician and/or an occupational disability employment counsellor. If this is the case, these appointments will be scheduled within 2 to 8 weeks of your application being sent.

    Decision

    You will receive our decision about your application within 14 weeks of you sending us your form. We will tell you whether we think you have the capacity to work and if you qualify for extra support. If you do qualify for extra support, we will inform you of what type of support you are entitled to and whether you will receive this support from us or the local council (‘gemeente’).
