If you want to apply for an unemployment benefit and top-up benefit at the same time, you can use the unemployment application form (‘WW-uitkering aanvragen’) to apply for both. You will need a DigiD account to do this.

If you are already receiving an unemployment benefit and want to apply for a separate top-up benefit, complete the top-up benefit application form (‘Toeslag aanvragen’).

If you are applying for, or receiving, an IOW or sickness benefit and want to apply for a top-up benefit, complete the top-up benefit application form (‘Toeslag aanvragen’). Please also use this form if you are not receiving an UWV benefit and are receiving a lower salary because you are ill.

You might also qualify for a top-up benefit when applying for, or receiving, a WIA, WAO, WAZ or Wajong benefit. If this is the case, complete the Top-up benefit application form when receiving an occupational disability benefit (‘Aanvragen toeslag als u een WIA-, Wajong-, WAO- of Waz-uitkering heeft’). You will need a DigiD account to do this.

• an overview of your total income

Please note: You must specify your partner’s income on the application form if you are either married or the equivalent of married (in Dutch). Your partner will have to sign this form as well. If you are single but sharing accommodation with a housemate, you might have to submit a copy of your tenancy contract as well.

You will receive a decision about the top-up benefit within 8 weeks of us having received your form.
