A return-to-work report is a complete set of documents that record what you and your employee have done to support your employee’s return to work. It should include the following documents:

  • the evaluation (‘probleemanalyse’)
  • the action plan (‘plan van aanpak’)
  • the first-year evaluation (‘eerstejaarsevaluatie’)
  • the most recent assessment by the occupational health physician or the occupational health and safety service (‘actueel oordeel bedrijfsarts of arbodienst’)
  • the final evaluation (‘eindevaluatie’)

You can upload the return-to-work report in the employer portal. You will need your eHerkenning account to do this. To make sure the WIA application is assessed as quickly as possible, please do the following:

  • upload the return-to-work report within 1 day of the employee applying for a WIA benefit
  • select the correct document in the document overview
  • sign each document 
  • check that the employee’s citizen service number (‘BSN’) is written on each document

Please note: You can upload the general documents. However medical documents are strictly confidential and must be submitted to us directly by the employee, and never by you.

We will send the employee a confirmation letter within 2 weeks of receiving the documents for the WIA application. You will also receive a copy. If the return-to-work report is incomplete, the letter will state which documents are missing and the deadline by which we need to receive them.
