To send us a short version of the return-to-work report, log in to the employer portal with your EH3 level eHerkenning account. Make sure you have the following information close to hand when completing the form:
- your employee’s first day of illness, their citizen service number (BSN) and any additional personal information
- your occupational health and safety service’s contact information (‘arbodienst’)
- your employee’s return-to-work information
You may want to re-hire your employee once they are able to return to work. If this is the case, let us know via the short version of the return-to-work report.
If an employee’s contract ends while they are ill, you will no longer be required to pay their salary. You will have to arrange a sickness benefit (‘Ziektewet-uitkering’) for them. To do this, inform us of your employee’s illness by using the ‘Report an absence’ feature (‘Verzuimmelder’) on the employer portal. Then select ‘Ziekmelding’ (‘Report an illness’) and fill in all the information required. Alternatively, if you are a Digipoort user, you can report your employee’s illness through your own HR administration system, which is directly linked to our system.
Do not forget to give your employee the return-to-work report when their employment ends. We will need this report when supporting your employee in their recovery process.
Please note: If you do not apply for a sickness benefit or if your application is late, you risk incurring a fine of up to €455.
